Please log into the Qualiphy portal and press on Clinics in your Dashboard

1. Follow the steps above to see the clinics web view, below.
2. For the clinic location you want to sign DocuSign for, click the three-dot icon located under the Actions column. Doing so reveals a number of options.

3. From these options, click Sign DocuSign.

4. This opens the DocuSign screen where the Qualiphy agreement is displayed for you to sign. This is explained in detail in the How to Register on Qualiphy? article.
